It is regularly happening when I use GPT-4.1.
It has occasionally occurred in the past with Claude Sonnet models but rarely.

Dominic R: Good to know! We're all using GPT-4.1 now too by default so hopefully will find it easier to see pop up. If you spot anything else that makes it more likely to appear let me know, will hopefully see it myself soon so I can squash it.

Which model(s) are you seeing this on? Generally would advise trying another model if you see this regularly, if you notice it's happening in specific model(s) more please let us know as it'll help in our efforts to reproduce (and fix) it!
